Asimismo, manifiesta dolor en la región umbilical de muchos años de evolución, que se mantiene según la paciente- de manera constante. Al examen, observamos una hernia umbilical, al parecer lobulada o dividida en dos partes. La hernia de la región externa mide 25.1 centímetros x 18.0 centímetros y la que ocupa la región media mide 12.0 centímetros x 10.0 centímetros. Laparoscopic hiatus hernia repair during LapBand surgery Hiatus hernia and gastroesophageal reflux is a common occurrence in patients undergoing obesity surgery. De dienstdoende arts die daarna voor haar verantwoordelijk is, gaat aan het einde van de dag naar huis. De verpleging belt de arts vier keer om te vertellen hoe slecht het met Esther gaat. Maar de arts komt niet. Esther krijgt infuusvloeistof om de lage bloeddruk te bestrijden. De verpleging gaat op eigen initiatief handelen. Zij vergeten te kijken hoeveel Esther uitplast en hoeveel vloeistof ze eerder toegediend heeft gekregen. Esther kan haar vocht niet uitplassen maar krijgt wel steeds nieuwe toegediend.Uiteindelijk overlijdt Esther 's nachts nadat haar vijf en een halve liter infuusvloeistof is toegediend. De doodsoorzaak luidt: 'overvulling'. De dienstdoende arts is door het Medisch Tuchtcollege berispt. In aflevering twee van Missers vertellen de ouders over het verlies van hun dochter en hun juridische strijd om de arts gestraft te krijgen. Een straf die volgens de ouders te laag is. Patient Education Once in place, the laparoscope will provide video images, so the surgeon can insert the instruments used to locate and pull back the liver... in order to see the upper part of the stomach. First, the surgeon pulls the stomach away from the hiatus, or opening, in the diaphragm. Then, the surgeon cuts away the tissue that connects the liver and the stomach. The surgeon can then dissect part of the diaphragm around the esophagus... This gives better access to the diaphragm and the esophagus. Next, the surgeon pulls the esophagus upwards... and closes the hiatus with sutures. Some hiatal hernias can cause persistent and painful acid reflux and your doctor may decide to correct the problem surgically. In this case, the surgeon divides and separate the arteries that supply blood to the top of the stomach. After freeing the stomach from the spleen, your doctor wraps the upper portion of the stomach around the esophagus and sutures it into place. Patient Education A rubber tube is placed in the esophagus to keep the wrap from becoming too tight. All of the instruments are withdrawn... the carbon dioxide is allowed to escape... the muscle layers and other tissues are sewn together and the skin is closed with sutures or staples.
