Impending death may be just a few breaths away-asthma. Death may be just a few breaths away-ASTHMA. The usually healthy 20-month-old patient was in respiratory distress and was about to decompensate at any moment. Was it Bronchiolitis or Status Asthmaticus (a common asthma emergency)? I made the clinical decision to treat him for status asthmaticus. The student of medicine should notice this patient's paradoxical breathing as well as his reduced level of activity. If an arterial blood gas had been performed and if it had revealed a pCO2 (carbon dioxide) greater than 45 mm Hg, it would have supported my working hypothesis that the child was in respiratory trouble (carbon dioxide retention is an ominous sign). This was a medical emergency. I immediately injected him with Epinephrine (aqueous) 1:1000 at 0.01 ml/kg/dose SC. His dose was calculated as follows: 20 pounds is equal to 9 kilograms (one kilogram = 2.2 pounds). 9 kg x .01 ml/kg = .09 ml (almost .1 ml; the dose I actually administered). After the epinephrine shot, I then set him up for treatment via a standard Albuterol solution via nebulization. The clinician must act promptly since the epinephrine has a short half-life and, in an emergency, a second dose may be required 15 minutes later. By administering the nebulized Albuterol via mask, a second epinephrine dose is usually not necessary. Albuterol is a more specific bronchodilator than epinephrine and is usually the preferred medication. As the child was receiving his Albuterol treatment, he was given 3 mg of Dexamethasone phosphate via injection (IM, intramuscular). The steroid treats the inflammatory component of asthma and begins to take effect an hour or two later. In summary, he promptly received short acting Epinephrine that was given subcutaneously, SC, in order to immediately begin to rescue him while I set him up to receive the Albuterol via nebulizer. Once improved, I began to give him apple juice in order to reverse his metabolic ketosis and provide him with hydration. An hour and a half later, he was no longer in distress and became very playful. Once I released him, I encouraged the mother to give him a bath once home in order to loosen his bronchial secretions and reduce the risk of bronchial plugging. In the United States, 4,000 to 5,000 asthmatics die yearly. None of my patients has ever died. When asthmatics present to the emergency room, they often get only partial treatment with a bronchodilator, Albuterol, and are sent home. A few hours after the Albuterol treatment, asthmatics rebound and can face a catastrophic death. By using a steroid, Prednisone or Dexamethasone, one treats the inflammation and prevents a serious relapse. Clinicians should not forget that asthma consists of bronchoconstriction and airway inflammation. In pregnancy, the corticosteroid of choice is not Dexamethasone because "it is not inactivated by placental 11ß-hydroxysteroid dehydrogenase." N Engl J Med 2003; 349: 776-788. Very few doctors know this information, and I am hoping that through this presentation it will become widely known. No pregnant patient with acute asthma should die as I have seen happen. All of my asthmatic patients have a two-day supply of a corticosteroid in case I am not readily available. Since I dedicate myself to treating the working poor and uninsured, I must make sure my patients are carefully educated regarding their asthma and how to treat it. I have been very impressed on how my patients have been able to safely treat themselves when I am away. In the end is not the dollar that excites me but rather the lives I save (life is priceless). Doctors must know what oxygen saturation means in terms of the actual oxygen dissolved in arterial blood (PaO2). On one very ill patient, for instance, an oxygen saturation of 81% may translate to a PaO2 of 45 mmHg (normal is more than 77 mmHg). I will briefly cover the topic of bronchiolitis, the alternative diagnosis mentioned that I had also first considered. Bronchiolitis occurs in the first 2 years of life and is usually due to a virus like the respiratory syncitial virus (RSV; the causative agent in up to 50%), Parainfluenza 3 virus or adenovirus. Mycoplasma is a bacterium that can cause bronchiolitis and is treated with a macrolide, an antibiotic. RSV, respiratory syncitial virus, is dreaded by many of us clinicians and often requires hospitalization. In terms of RSV, you should visit the cdc.org web site for more of the latest information on how to prevent and treat RSV bronchiolitis. The other topics in this video include laboratory data. Clinicians must again be able to readily use the microscope as I do. A physician cannot confuse a urinary white blood cell with Trichomonas vaginalis. Trichomonas vaginitis is a sexually transmitted. However until seeing the dinner plate example described here, I have always tended to think of galaxy's as being far further apart in relationship to their sizes, indeed after seeing this, the famous Hubble Deep Field galaxy photograph now makes far more sense to me. .
