Mishap of B-52 at Fairchild Air Force Base Washington Please try to keep from using vulgarities and profanity. I feel we can make our points without using profanity. I have no problem with calling the pilot a jerk, however remember there can be children viewing this thread so try to keep suitable for public. US air show standards are the highest, but this was a case of systemic disregard for safety. Since this crash, the bar has been set higher more enforcement and awareness. It bothers me to see some other counties still have very poor safety at major air shows. The former soviet block has had several crashes that I would consider safety neglect a factor, if not the cause. Aerobatics should not be done over crowds. Aerobatics should be kept in a safe margin away from the crowd and to some extent the flight line. I even frown on extremely low passes over crowds. Pilots and management/command should have restraint from flying planes to extreme limits at air shows. A margin / buffer zone should be between the aerobatic area and the crowd. The B-52 was just practicing; if he had crashed in a crowd at an air show the loss of life would be high. The B-52 did some low high speed passes over personnel area and flight line; if there was a failure, he could have possibly destroyed all the aircraft on the flight line (and equipment, buildings) or and killed a lot more personnel. I love aircraft and air shows and the US. I hate to see a bunch more rules, I would prefer pilots and people in charge do there best to keep things safe with knowledge, reasonable action. I think the US and many other countries have learned from this accident to take air show safety more rationally. However, a few air shows like to try to out perform other air shows with unsafe flashy showboat shows. When you push the limits you tend to exponentially increase the risks. If a pilot makes a goof or if there is a failure, with buffer zones the risk to the crowd can be minimized. --- On 24 June 1994, a B-52H of the 325th Bomb Squadron, 92d Bomb Wing at Fairchild Air Force Base (AFB), Wash., crashed while preparing to land after practicing maneuvers for an air show, killing all four crewmen. The pilot in command had over a long period of time demonstrated a disregard for Air Force flying rules and regulations, and this was known by the senior commanders in the wing. No appropriate action had been taken to discipline him or rein in his noncompliant behavior. Super STOL C-130 Mishap I have seen this video several places, many don't tell the story behind it, and many seem to have it wrong. Operation Credible Sport, also known as Operation Honey Badger, was a United States military operation plan in late 1980 to rescue the hostages held in Iran using C-130 cargo planes modified with rocket engines. The Credible Sport operation was to follow the dramatic failure of Operation Eagle Claw in which a C-130 Hercules and a Sea Stallion helicopter collided in the Iranian desert, killing 8 servicemen. Credible Sport was abandoned after the election of Ronald Reagan as President in November, 1980. The Credible Sport plan called for highly modified C-130 Hercules cargo planes to land in a soccer stadium not far from the American Embassy in Tehran and airlift the hostages out. Three aircraft were modified under a top secret project at Eglin Air Force Base, Florida to YMC-130H configuration with rocket packages blistered onto the forward and aft fuselage, which theoretically enabled the planes to land and take off within the confines of the sports arena. During a demonstration at Duke Field, Eglin Auxiliary Field 3, on October 29, 1980, one of the modified Hercules fired its braking rockets a few seconds early (this seems to be wrong, see note below). The aircraft suffered an extremely heavy landing, tearing off the starboard wing, setting off a fire, and resulting in the airframe, serial 74-1683, being written off. Despite this, the entire crew survived. This failure, coupled with the defeat of Jimmy Carter by Ronald Reagan in the presidential election on November 4, 1980, led to the cancellation of this rescue mission plan. The hostages were subsequently released concurrent with Reagan's inauguration in January 1981. The other two airframes, serials 74-1686 and 74-2065, were stripped of their rocket modifications and returned to regular airlift duties. In 1988 74-1686 was placed on display at the Robins Air Force Base museum, Georgia. As of 2005, 74-2065 is assigned to the 317th Airlift Group, 15th Expeditionary Mobility Task Force, at Dyess Air Force Base, Texas. Note One of the test pilots during the Credible Sport program elaborated on the test failure, the test with the footage now available. The pilot states that during the test the braking rockets did NOT fire prematurely, but right on time. It was the descent-arresting rockets, rockets that were supposed to fire after the braking rockets to slow the drop of the aircraft as the aircraft's forward velocity was halted and the wings ceased to provide lift that failed to fire. Looking at the video, it would appear that the braking rockets should be fired after the aircraft touched down to quickly stop the landing roll. However, the braking rockets were supposed to be fired while in the air, then the descent arresting rockets would fire to allow a controllable and almost vertical landing. It was these rockets that failed, with the crash landing and aircraft breakup resulting.. Firework Mishap - Thousand Oaks, CA GOOGLE MAPS LOCATION: (enter this in Google Maps) 34.185742, -118.880298 What I meant by "THIS IS COOL": This WAS "cool" because I knew that the firefighters had it under control before it started, but NOT... "cool" because I wanted to see it get out of control. The vegitation on the hill was cut from the middle of the hill down and firefighters were covering all sides of the hill just in case something happened. I was a firefighter myself before I became a full time film maker, so I understood the procedure. Anyone leaving me hateful comments about saying "This is cool!" is just prejudgement and too ignorant to read this description... and THAT is NOT cool. Stay positive, because this video wasn't suppose to be any other way. This was just MY experience on 4th of July and NO ONE was hurt. WHAT HAPPENED? During a city fireworks display on top of a hill that we call "Tarantula Hill", a firework shell misfired and landed in some near by vegetation. Luckily, the firefighters were prepared and the fire was quickly put out. Since I'm always armed with a camera, I had to share the moment while it lasted, but unfortunately I didn't capture the flash. I shot this video from the roof of my home. In times like this, I sure miss being a firefighter! USS Forrestal Mishap July 29, 1967 Lately there is a political conspiracy to smear, slander and defame McCain. As the evidence clearly shows it would be absurd to try to blame McCain. With the political primaries there has been a figurative political assassination attempt to slander McCain by saying an insinuating that McCain murdered his crewmen by deliberately causing this mishap. The facts and evidence clearly shows that McCain was a victim and not responsible for this mishap. The real experts agree, the only "experts" that disagree are clearly politically motivated since the self-proclaimed "experts" bashing McCain didn't pop up until the election started heating up. I find it despicable that these hateful political activists resort to slandering McCain a military veteran POW in good standing. I have mixed feelings about McCain's politics. Regardless on how you feel about his politics you should respect the man for volunteering to go into harm's way to serve the military and his country and for being a POW and suffering torture that most people couldn't fathom. I think these political activists that are spamming their slander should spend eternity in hell. And I have seen a few documentaries on cable TV that has SOME of this footage. This tape is old and damaged. The video is poor but audio/narration is good. There is some better video out there but I think they have more film cut out.Some clipped and edited differently. The rocket screamed across the flight deck, struck another aircraft and ignited a fuel fire. The initial fire could have been contained, but 90 seconds after the fire started a bomb detonated, killing or seriously wounding most of the fire fighters. The detonation ruptured the flight deck, and burning fuel spilled into the lower levels of the ship. Bombs, warheads, and rocket motors exploded with varying egress of intensity in the fire, killing 134 and wounding 161 men. Twenty-one aircraft were destroyed After this incident, the Navy established a flag level committee to pursue improvements to the systems used to control flight deck fuel fires. An ordnance safety program was also initiated to characterize flight deck fuel fires and study ways to delay the "cook-off" times of munitions. Aircraft Mishap Montage 1 part 1 The F-16 was only on a high-speed taxi test, but a control induced oscillation became so bad he opted to take it off. Some of the sources say the YF-16 (control oscillation) did not have a computer, I do not think that is totally correct. I think it had a computer, however it was a very primitive computer and did not have any software to prevent oscillations. After this mishap I think they upgraded the computer and software and did some other modifications to reduce the chances of control induced oscillations. I think also they developed computer flight simulators to get pilots used to flying by wire and the new style control stick. With most aircraft you can tell how far the controls are lagging behind because the controls are directly linked, however with flyby wire and some versions of hydraulic control there is some lag that can easily confuse a pilot into over controlling the aircraft. If you try to fly an airplane that has a lot of control authority lag, you have to dampen your control inputs, and that does not come naturally. "Philip F. Oestricher was the first pilot to fly the YF-16. This occurred on 20 January 1974 during hi-speed taxi trials. He was a civilian employee of General Dynamics at the time" John G. Williams, a structural flight test engineer on the YF-16, recalls: During the first high-speed taxi test, a violent lateral oscillation had set in as a direct result of pilot-commanded oscillations (several maximum left/right commands) as the airplane reached rotation speed (~120 kt). Remember, this was the first airplane to have a fixed stick, and there was no opportunity for Phil to gain any feel for the airplane, until that high-speed taxi test. As the nose of the aircraft rose, the tailplane inadvertently scraped on the runway. The left wingtip missile and the right tail static probe also lightly contacted the runway. Phil chose to take off because the bird had begun to veer off to the left side of the runway, and he was faced with plowing through the desert or flying. Thankfully he chose to fly and possibly saved the entire program. After take off, Phil regained control and stayed up for six minutes, and landed uneventfully. Prior to the next flight, the stick sensitivity was reduced by 50% with gear down. Later, after complaints of not enough sensitivity, it was returned to the original." "The prototype YF-16 (serial number 72-1567) was rolled out at Fort Worth on December 13, 1973. It was powered by a Pratt & Whitney F100-PW-200 (JTF22A-33) turbofan, rated at 23,830 with full afterburner. Maximum takeoff weight was 27,000 pounds. The YF-16 carried no radar, and the aircraft had analog flight controls with no computer software. The YF-16 was air freighted by C-5A to Edwards AFB on January 8, 1974. Its first flight was an unintended short hop around the pattern on January 21, 1974 at the hands of test pilot Phil Oestricher. During high-speed ground tests at Edwards, Oestricher had inadvertently scraped the tailplane on the runway as the nose was raised, and a violent lateral oscillation set in. He decided to take off and regain control in the air. He stayed up for six minutes and landed uneventfully. The scheduled first flight was delayed until a new right stabilator could be fitted. The first official flight took place February 2, 1974, again with Phil Oestricher at the controls. He reached 400 mph and 30,000 feet." Good subject; poor video quality.
